Earliest: 11 weeks before your due date. Latest: 6 months after the baby's birth. Don't miss this window - late claims cannot be accepted.

How to apply

Download and complete form SF100 from gov.uk. You'll need a statement from your midwife or health visitor (Part 2 of the form) confirming they've given you health and welfare advice. Post the completed form to your local Jobcentre Plus. You can also call the claim line on 0800 055 6688.

  • This is only for your first child unless you're having multiples - many parents don't realise this
  • The midwife/health visitor statement must be completed after you're 29 weeks pregnant or after the birth
